Games Forms

Below are the forms that you will need in order to register your athletes and coaches for the 2010 Games. Please print out the forms and complete them entirely. If you have any questions on the forms, please do not hesitate to contact me at

Health Forms need to be filled out completely by both athletes & coaches.


 
 

All athletes and coaches (preferred for volunteers also) must be members of Michigan Disability Sports Alliance in order to participate in Games. There are 3 levels of membership and all memberships will need to be paid in full in order to complete Games.


 
 
 

Photo release forms must be completed and signed by all athletes and coaches in order to compete in Games. Your signature of such gives us the authorization to take, keep and use photos and videos to promote appropriately for sanctioned Games/ organizations. If athletes and/ or coachs are under 18 years of age, a parent or guardian will need to sign also.


A registration form on EACH of your athletes must be completed in order to compete in the games. Please review entirely. If your athlete has Cerebral Palsy (CP), please choose either the junior form (under 18 years of age) or the senior form (over 18 years of age) and complete. If your athlete has a different disability other than CP(i.e. Multiple Sclerosis, Muscular Dystrophy, Spina Bifida, etc.), please fill out appropriate Les Autres (LA) form.
